Policy/Privacy Statement

Physicians Choice Private Duty is concerned with offering comprehensive in-home assessments for seniors to assure a safe living environment.

Physicians Choice Private Duty assists clients in making informed choices. We may provide information which includes (but not limited to) programs and service providers and suggested courses of action. Physicians Choice Private Duty makes no promises, warranties, claims, guarantees or endorsements regarding this information or any referrals made by the staff of Physicians Choice Private Duty. All final decisions resulting from the assessment process rest with the client.

To safe guard the privacy of clients, Physicians Choice Private Duty has adopted the following policies:

1. Physicians Choice Private Duty holds in strict confidence all client information regardless of the source or sources from which it is received.

2. Physicians Choice Private Duty does not accept assessment requests by anyone “on behalf” of consumers except for persons who are legally empowered to represent such consumers. All services are voluntary.

3. Physicians Choice Private Duty does not provide information on assessments, progress or outcomes of any case to anyone without the client’s prior written permission.

4. Physicians Choice Private Duty is required to report any instances of “Elder Abuse” defined as “any knowing, intentional or negligent act by a caregiver or any other person that causes harm or a serious risk of harm to a vulnerable adult” and may include abuse that is physical, emotional, sexual or financially exploitative.
